Remember why the decision was made.
Two years on, somebody asks why the system works this way. The people who decided have moved on, the discussion was in a chat that has since been migrated, and the only surviving artifact is the code, which records the conclusion and none of the reasoning.
Return with context.
The next person can see the reason.
The situation
Groups do not usually fail to make decisions. They fail to keep them. The decision is made in a meeting, refined in messages, implemented in a change, and the reasoning evaporates. What remains is a system full of choices nobody can explain and therefore nobody dares to change.
How this usually goes
The rationale exists in one person's memory, which is a single point of failure with a notice period.
A written record exists, is out of date, and is trusted anyway, which is worse than none.
Work interrupted mid-way cannot be resumed by anyone else, so it restarts instead.
The same question is re-litigated every eighteen months at full cost.
What happens here
1. Multi-step work is carried by an ordered ledger
Not a task list. Each step states what will be done, how it will be checked, and what happened, and it is marked with its real status rather than an optimistic one.
2. The check is written before the work
Deciding in advance what would count as done removes the most common failure in both human and delegated work, which is declaring success and moving on.
3. Outcomes record what was found, including the surprises
A step whose outcome contradicts its plan is more valuable than one that confirms it, and it is written down rather than quietly reconciled.
4. Decisions are separable from the narrative
A choice with real alternatives is recorded as a decision with its options and its reason, so it can be found later without reading the whole history.
5. Anyone can resume
Because state, reasoning, and next check all live in one ordered document, work interrupted for a week or handed to someone else continues rather than restarting.
What the community keeps
The ability to change its mind safely. A group that can see why it chose something can revisit the choice when conditions change, which is the difference between a system that evolves and one that ossifies until someone rewrites it.
